ADJUNCT INSTRUCTOR APPLICATION MATERIALS

The following materials are required for a complete adjunct application: WTCS Certification Application, Resume, Official transcripts - Graduate and Undergraduate (not copies), Appropriate non-teaching work verification,W-4 forms, I-9 form, and Direct Deposit form.

Please PRINT ALL OF THE FOLLOWING FORMS and read CAREFULLY:

Ø WTCS Certification Application (This form must have a signature to be a valid application.)

Ø WT-4 form (Wisconsin State Tax form. You must have Adobe Acrobat Reader* to view)

Ø W-4 form (Federal Tax form. You must have Adobe Acrobat Reader* to view)

Ø I-9 Instruction Sheet and I-9 form (YOU MUST PRINT INSTRUCTIONS AND FORM! This form MUST be completed on campus! You must have Adobe Acrobat Reader* to view)

Ø Verification of Employment

Ø Direct Deposit form (You must have Adobe Acrobat Reader* to view)

Ø Transcript Request form (You must have Adobe Acrobat Reader* to view)

For individuals who live in Illinois, please print the following:

Ø Illinois W-4 Instruction Sheet and W-4 form, (You must have Adobe Acrobat Reader* to view)
